What is the covering Letter for Good Standing?

The covering letter for Letter of Good standing should contain following details

a)    Name, full address, Land line / Mobile number and Email ID of the applicant
b)    Letter to should be addressed to General Secretary, The Indian Association of Physiotherapists
c)    Mention the candidate’s IAP Life Membership number and reason for which the Letter of Good Standing requested
d)    Name of the Country / Authority the candidate applying for and the purpose 
e)    Full address with phone number to where the “Verification / Letter of Good Standing” to be sent
f)    If you require only soft copy please mention the email ID to which the Letter of Good Standing to be sent
g)    Please mention whether this first time or similar verification done by IAP earlier if so please mention the details.
